I downloaded the theme creator from Sony Ericsson for my W580i. I can't figure out if there is a way to extract an image from a previously created theme. Does anyone have any idea?

Yes you can, say that you're going to create a new theme file: New.thm, but you want something from the old theme file: Old.thm. Take the Old.thm file and open it with winrar or tugzip [both free applications] then you will see just a list of files that were used in the theme. Since you know the name of the file in Old.thm that you want in New.thm, then just drag that to the desktop and insert it into the New.thm that you've got open in the SE theme creator.
